PATIENT PRE-REGISTRATION
You can reduce your check-in time by downloading
the Kearney Clinic Patient
Information form*. Please fill out the form as completely
as necessary and bring it with you to the Kearney Clinic.
Please DO NOT SIGN
the form until
it can be witnessed by a Kearney Clinic employee.
Upon your arrival at Kearney Clinic, P.C. you will be
asked to verify your registration information, a copy
of your insurance card will be made, and you will be asked
to sign required authorization forms.
The New Patient Information form is not an appointment
schedule. If you wish to submit a request for an appointment,
you may do so on the Appointment
page. |
